Position Summary

About the Position: The Permanent Housing Advanced Clinical Services Team (PHACS) brings medical and behavioral health services to clients in Permanent Supportive Housing (PSH) sites throughout San Francisco. The team also provides consultation, coaching, and training to the on-site contracted providers, all in an effort to improve access to care and to bridge services into ongoing solutions and connections to longer-term service support. This position will allow the program to expand to serve more clients, to create feedback loops with clients and on-site case managers, and to assist in the quality improvement efforts of the program. The AmeriCorps member's duties will include direct care coordination services supporting PSH residents, and working with team members to improve processes. Further, the member will work with on-site case managers to increase the capacity of the team.

About the Organization: SFDPH Whole-Person Integrated Care serves to provide care to people experiencing homeless by 1) connecting to transitional primary care 2) re-/connecting to social services 3) supporting client’s towards becoming “housing ready” for their transition from shelter health to their next destination, ideally their own permanent, supportive housing with established primary medical care.

Major Duties and Responsibilities

  • Refer patients to other programs and community services
  • Track patients to confirm program enrollment and engagement
  • Provide follow-up and case management to deliver assistance and information
  • Provide social service navigation
  • Conduct program quality improvement and data analysis
  • Accompany clients to appointments
  • Provide logistical support for on-site case manager training needs
  • Conduct surveys and/or interviews to improve programs
  • Attend staff meetings and other relevant meetings
  • Document work in electronic health record

  • Note from host site: PHACS is a relatively new program and is continually improving services and workflows. Flexibility, patience, the ability to problem solve, and research and analytical skills are key to success.

Knowledge Required for the Position

  • Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.

Supervision

  • The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required

Review

  • Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.
  • The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service.
  • Note from Host Site: The position will have the support and coaching from the PHACS Program Coordinator and Nurse Manager to ensure proper onboarding, program and position training, and successful completion of tasks/projects, as well as the supervision of the Host Site Supervisor.

Special Considerations

Member will:

  • Serve Monday-Friday 8:00am-5pm
  • Serve Approximately 36-40 hours per week
  • Significant travel (more than 25%) between clinics and service sites
  • Unsafe or hazardous surroundings or working conditions
  • Additional health screening or service required
  • Note from Host Site: PHACS is a mobile program in which team members visit clients in their homes. Our work often involves visiting clients' homes in high-crime areas. We may encounter biohazardous waste, sharps, vermin, and cluttered belongings in the home environment. Many of our clients use substances and/or have mental health diagnoses. We offer a supportive team and office environment to help our staff cope with experiences related to the work. There are 145 Permanent Supportive Housing buildings.  Most team members walk and/or access public transportation.
