Position Summary

The member will serve with the Family Practice & Counseling Network's (FPCN) prenatal program. Our team, comprising providers (Physicians and NPs) from the University of Pennsylvania, a prenatal nurse, and a medical assistant, aims to provide comprehensive trauma-informed care to FPCN’s OB patients based on current clinical data. The member will partition their time across 3 clinics in which FPCN offers prenatal care for its patient population. The size of the patient population the member will manage usually varies from ~25-40 on a weekly basis. Pregnancy requires substantially greater engagement in care than in most other periods of life. Thus, existing barriers to accessing care are compounded, putting patients at greater risk for complications that affect both themselves and their babies. Because we serve a population greatly affected by race, poverty, and other social determinants of health, we engage in extensive efforts to encourage and support compliance with care. As a Federally Qualified Health Center (FQHC), our program is evaluated based on this ability to engage patients in care – from the first trimester through the postpartum period. In addition, we are evaluated on our ability to support pregnancy to at least 37 weeks, with the goal of infant birth weights >= 2500 grams.

Major Duties and Responsibilities

Following-up with patients to ensure documentation is completed. Assisting in care coordination and resource referrals. Conducting outreach to patients via phone/letters.
