Position Summary

Greater Philadelphia Health Action, Inc. (GPHA) is an FQHC dedicated to providing medical, dental, and behavioral health care for people regardless of their ability to pay.  They serve over 85,000 patients per year, many of whom experience chronic disease conditions and/or behavioral health conditions that need attention. Many GPHA patients need support to get to their appointments due to social barriers to healthcare.

Community Health Workers (CHWs) play an important role in GPHA's health care model. They support patients before the visit to schedule appointments, get referrals, and complete lab work. CHWs help patients navigate digital health tools and resources like the patient portal, and can be available to assist patients between visits with their provider. CHWs also help patients get the care they need to close care gaps like immunizations, blood glucose control, and blood pressure control, which are important for supporting patient health and well-being.

Major Duties and Responsibilities

The NHC member will serve as a Community Health Worker (CHW) for patients and residents of North Philadelphia.  

The NHC member will outreach to North Philadelphia residents, using a population health database to identify patients in the target area, assess needs for primary care medical, dental, or behavioral health, or pharmacy services, and assist residents to get connected to care at the new Hunting Park Health Center.

The NHC member will also assist residents in scheduling appointments or lab services, help residents/patients navigate the patient portal by setting up an account, logging in, and printing/downloading information, and outreach to North Philadelphia patients who were transferred to another site when the Hunting Park site was closed for renovations.

The NHC member will also support the process of identifying care gaps  (through completion of the PRAPARE assessment for social determinants of health) and contacting patients with patient education, resources, and scheduling support, as well as participate in planning and implementing community outreach events targeted for the 19140 zip code.

Knowledge Required for the Position

Supervision

  • The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required

  • Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards, and files from previous projects

Complexity

  • The facts or conditions of the service position are clear cut, predictable, and apply directly to the assignment

Special Considerations

  • Special safety regulations or precautions that must be observed

  • Criminal history check requirements beyond those required by National Health Corps
  • If working in a clinic,  the member must observe any special safety precautions that are in place (i. e. PPE).
  • Employee health screening required including TB and Hep B.
