Position Summary
2. Conduct community outreach, develop/administer surveys, and conduct patient interviews to identify persons at risk of physical and mental health issues at local venues
3. Create and distribute health-related print materials to the community and also social media content
4. Administer annual community needs assessment
5. Connect constituents to Family Life Project's healthy lifestyle coaching services
6. Assist with providing services within the Family Life Project's healthy lifestyle coaching services
7. Refer constituents to additional community resources that will promote wellness goals.
8. Follow-up with constituents via phone or telehealth to mark progress
Characteristics of an Ideal Candidate
Team-oriented, Flexible, Organized, Multi-tasker; able to balance, Comfort speaking with others, individually or in groups
Knowledge Required for the Position
Knowledge of AmeriCorps/Health Corps member requirements, Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.
Supervision
The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.
Review
The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service., Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations.
Guidelines
Written and oral guides provide specific instructions for doing service, Guidelines are generally applicable, but the member independently makes adaptations in dealing with problems and unusual situations
Complexity
The facts or conditions of the service position are clear cut, predictable, and apply directly to the assignment
Special Considerations
Unusual service hours, involving evenings, weekends, or long shifts, Personal vehicle required, Significant travel (more than 25%), Criminal history check requirements beyond those required by National Health Corps
