Position Summary

About the Position:

The Ampla Health Care Coordinator NHC Member will serve within Ampla Health’s Ryan White HIV Program and will support a variety of public health, community outreach, and quality improvement initiatives. The NHC Member will assist with data collection and reporting for program measures, support updates to the Ryan White 5-Year Plan, and help coordinate HIV focus groups that contribute to community planning and program development. The NHC Member will also participate in community needs assessments to help identify barriers to care and improve access to services for under-served populations.

In addition, the NHC Member will assist with outreach events throughout the community, including health fairs, education events, and prevention activities focused on increasing awareness of HIV services, healthcare access, and available community resources. The NHC Member may also support emergency preparedness activities, patient engagement efforts, health education initiatives, and coordination of community partnerships.

Finally, the NHC Member will participate in Ampla Health’s Service Excellence Initiative (SEI), which focuses on improving patient experience, communication, teamwork, and quality of service across programs. 

About the Host Site

Ampla Health is a 501 (c)3 non-profit network of community-based Federally Qualified Health Centers (FQHC) offering comprehensive medical, dental, behavioral health, pediatrics, chiropractic and specialty healthcare services in Butte, Colusa, Glenn, Sutter, Tehama and Yuba counties. Ampla Health has fourteen medical and six dental centers conveniently located throughout Northern California to provide patients and their families greater access to their health care needs.

Major Duties and Responsibilities

  • Providing follow-up and case management to deliver assistance and information.
  • Maintaining professional boundaries and respecting patient confidentiality.
  • Updating documentation to assess program adopting and process.
  • Collaborating with supervisors and other leadership.
  • Conducting program quality improvement and data analysis.
  • From the Host Site: The position will also include:
    • Data collection and reporting for program measures
    • Updating the Ryan White 5-Year Plan
    • Coordinating HIV focus groups as part of the 5-Year Plan process
    • Supporting emergency preparedness activities and community needs assessments
    • Assisting with outreach events and community engagement efforts
    • Participating in Ampla Health’s Service Excellence Initiative (SEI)
    • Performing other duties as assigned by the supervisor.

Knowledge Required for the Position

  • From Host Site:
    • The position requires strong communication, organization, and teamwork skills.
    • The member should be comfortable using Microsoft Office programs such as Outlook, Word, Excel, and Teams for communication, data tracking, scheduling, and reporting activities.
    • The member should also be able to maintain confidentiality, interact professionally with diverse populations, and support outreach and community engagement activities.
    • Basic knowledge of healthcare, public health, or community services is preferred but not required, as training and ongoing supervision will be provided.

Supervision

  • From Host Site: 
    • The member will participate in regular communication and daily team huddles to discuss schedules, outreach activities, program needs, questions, and workflow coordination. 
    • As the member gains experience, they will be expected to use initiative in carrying out recurring assignments while continuing to work under the guidance and supervision of program staff.

Review

    • From the Host Site: 
      • Projects and assignments will be reviewed by supervisors to ensure objectives are met and work is completed in compliance with organizational policies, procedures, and program requirements. 
      • As the member gains experience, they may independently carry out recurring assignments while continuing to receive guidance and support from supervisory staff.

Guidelines

  • Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards, and files of previous projects.
    • From the Host Site:
      • The member will follow written and verbal guidelines provided by Ampla Health, including organizational policies and procedures, program requirements, confidentiality standards, outreach protocols, and supervisor instructions. Detailed guidelines and training materials will be available to support the member in completing assigned duties and responsibilities.
      • As the member gains experience, they may use independent judgment in carrying out recurring assignments and adapting to routine situations while continuing to work within established policies, procedures, and supervisory guidance.

Complexity

  • From the Host Site:
    • The member’s assignments may involve a variety of tasks related to outreach, community engagement, program support, and administrative responsibilities. While many assignments follow established procedures and organizational guidelines, the member may also need to analyze information, adapt to changing schedules or community needs, and use judgment when responding to routine questions or situations.
    • The position may involve varying assignments and occasional unusual circumstances related to outreach events, community engagement activities, and program support efforts. The member will work under supervisory guidance while developing problem-solving, communication, and organizational skills throughout the service term.

Special Considerations

  • Due to the wide service area that Ampla Health covers, the NHC Member must possess a valid driver’s license.
  • Dress Code:
    • Monday through Thursday, the NHC Member may wear scrubs or professional attire, including nice slacks and a blouse or professional shirt. Open-toed shoes are not permitted.
    • On Fridays, the member may wear jeans or casual pants without holes. Shirts may be worn as long as they do not contain offensive language, inappropriate content, or gang-affiliated symbols or imagery. Facial piercings are not permitted, with the exception of ear piercings.
  • Unusual Service Hours
    • From Host Site: Most outreach activities take place during the evenings or on weekends. 

Language Proficiency Requirements

  • No language other than English is required.
    • Proficiency and skill with Spanish is highly desired to communicate with a wide range of people (patients, clients, community members, etc.)

Criminal History Check Requirements Beyond NHC Standard Checks

  • In addition to standard NHC Background Checks and Medical Exam, the NHC Member will:
    • Be required to complete a TB test, physical examination, and reflex drug screening.
    • All required screenings and testing required beyond NHC's Medical Exam will be paid for by Ampla Health.
