Position Summary
About the Position:
The QI/Diabetes Care Coordinator NHC Member will support HealthRIGHT360's Clinic Operations Supervisor, Nutritionist, and Medical Director in implementing Quality Improvement for our diabetic patients in addition to patients in our care who we have lost to follow up.
The NHC Member will serve with the HealthRIGHT 360 Nutritionist in our weekly Food Pharmacy to provide healthy foods to under-served patients. The NHC Member will be a part of the team to serve on targeting specific Quality Improvement projects for patients with chronic conditions members, including diabetes, heart disease, hypertension, and others. The NHC Member will be supported by the Medical Director, QI Manager, Nurse Manager and Clinic Operations Supervisor to implement quality improvement projects.
The NHC Member will also actively participate in our Hospital Discharge processes, providing outreach to patients who discharge from the hospital and help in scheduling follow-up appointments with their primary care provider within 7 days of their discharge date.

Our supportive staff is experienced with the complex healthcare needs that can result from homelessness, poverty, and addiction. Our providers have specialized expertise in preventative health, chronic conditions, the treatment of infectious diseases, and addiction.
Major Duties and Responsibilities
- Tracking patients to confirm program enrollment and engagement.
- Coordinating health and wellness activities including classes and groups.
- Providing social service navigation.
- Maintaining, assembling, and distributing materials and supplies.
- Conducting quality improvement and data analysis.

Knowledge Required for the Position

- The NHC Member will:
- Serve Monday - Friday, 8:30AM - 5:00PM
- Serve approximately 40 hours per week at their Host Site
- Serve in-person
- Dress Code: No graphic or political tees, and closed toed shoes required.
- Personal vehicles are not required for this position. The clinic(s) are accessible via public transportation.

- Upon Conditional Acceptance of an NHC Position, all NHC members must complete TrueScreen Background Checks to be formally placed in the NHC Program.
