Position Summary

About the Position:

The SFHN Care Coordinator - Overdose Prevention and Value-Based Care NHC Member will support and implement quality improvement initiatives across primary, specialty and inpatient units. They will help with telephone outreach, data analysis, and develop standard work to help improve health outcomes. The focus of this hybrid role is to focus on SFHN's priority on Opioid Prevention and providing Value-Based Care to SFHN's patients. 

San Francisco's commitment to improving healthcare quality has led to significant improvements in chronic disease management, preventative screenings, and immunization rates. The NHC Member will support these initiatives by assisting with patient outreach, data review, and process improvements to enhance healthcare access and efficiency. Additionally, San Francisco has seen a rise in opioid-related health concerns. Clinic staff have implemented safety measures for patients prescribed opioids and expanded treatment options for opioid use disorder (OUD). The NHC Member will support patients seeking treatment by facilitating access to care and coordinating necessary resources.

About the Host Site:

The San Francisco Department of Public Health (SFPDH) strives to achieve its mission of protecting the health and safety of all San Franciscans through the work of two Divisions - the San Francisco Health Network (SFHN) and Population Health and Prevention. The San Francisco Health Network (SFHN) is the City's health system, with locations throughout the city, including San Francisco General Hospital Medical Center, Laguna Honda Hospital, and over 12 primary care clinics. The SFHN serves approximately 64,000 patients, the majority of whom are publicly insured or uninsured, and may require additional health literacy support. The member will engage with patients and staff from a variety of socioeconomic backgrounds.

Major Duties and Responsibilities

  • Identifying patients who are at-risk to assess eligibility to various services.
  • Providing follow-up and case management to deliver assistance and information.
  • Auditing charts for quality assurance.
  • Documenting interactions with patients in an electronic health record.
  • Conducting program quality improvement and data analysis.

Knowledge Required for the Position

  • Knowledge of AmeriCorps/Health Corps member requirements
  • Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options. 

Supervision

  • The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required
  • The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues. 

Review

  • The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service.
  • Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations. 

Guidelines

  • Written and oral guides provide specific instructions for doing service
  • Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards, and files of previous projects 

Complexity

  • The facts or conditions of the service position are clear cut, predictable, and apply directly to the assignment

Special Considerations

  • The member will:
    • Serve approximately 40 hours per week.
    • Serve Monday-Friday from 8:00AM - 5:00PM.
    • The SFHN Care Coordinator is required to serve onsite 4 times a week with flexible hours from 8am - 5pm, allowing for remote service 1 time(s) per week.
  • Personal vehicles are not required for this position. The clinics are accessible via public transportation.

Language Proficiency Requirements

  • No language other than English is required.
    • Proficiency and skill with Spanish, Cantonese, and/or Mandarin would be useful to the member to communicate with a wide range of people (patients, community members, administrators, parents, etc.).

Criminal History Check Requirements Beyond NHC Standard Checks

  • Upon Conditional Acceptance of an NHC Position, all members must complete TrueScreen Background Checks to be formally placed in the NHC Program.
  • No Background Checks or Health Exams required beyond what is required of NHC Members.
