Position Summary
The member will serve on the Outreach Team to promote EDCHC services, helping to increase patient capacity and address barriers to care with a focus on high-risk populations.
The member will:
Conduct in reach assessment to promote client healthcare services and address barriers to care with a focus on no-show and high risk patients. Contact patients in need of various services, like dental, eye care, cancer screenings, diabetes checkups, hypertension, etc. Assist with event planning for fundraising, employee engagement, and outreach to the community including booth events and health fairs.
Major Duties and Responsibilities
Providing outreach to agencies to build awareness
Creative and developing communications related to outreach campaigns
Developing and providing health education for the community
Providing health education to at-risk populations
Attending trainings and/or community events
The member will be offering additional support to the Administrative Services Manager and other departments, who are already responsible for the job duties included.
