Position Summary

This position will serve as the NHC Service Member with the purpose of analyzing the conditions of our aging patient population and how One Community Health can better meet the needs of this patient population. This analysis is critical for our patients over the age of 50, particularly for those patients that are low-income, have co-morbidities, are HIV-positive, and/or face unstable housing situations.

Major Duties and Responsibilities

  • Providing follow-up and case management to deliver assistance and information
  • Implementing patient engagement strategies to improve outcomes
  • Coordinating health and wellness activities including classes and groups
  • Educating patients on medical interventions and prevention
  • Maintaining, assembling, and distributing materials and supplies
  • Developing new program or services to address community needs
  • Developing/implementing surveys, focus groups, interview guides, observations, or other data collection tools to identify opportunities for improvement in service delivery
  • Developing communications messages/strategies
  • Creating tracking or data management systems
  • Generating and disseminating reports

Characteristics of an Ideal Candidate

Knowledge Required for the Position

Skill with Microsoft Office or other software for a variety of data processing operations involving a range of problem solving, record keeping, correspondence, and service tracking options.

Supervision

The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required, The supervisor assigns service activities in terms of project objectives and basic priorities and is available for consultation in resolving controversial issues.

Review

The member independently plans and carries out the projects and selects the approaches and methods to be used in solving problems

Guidelines

Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards, and files of previous projects

Complexity

The service position assignments vary significantly or often involve unusual circumstances and incomplete or conflicting data
