Position Summary
The AmeriCorps member will serve within FPCSN’s Community Engagement and Health Wellness initiatives, a network-wide effort designed to strengthen connections between health centers and the communities they serve. This initiative focuses on expanding outreach, enhancing health and wellness programming, and addressing social determinants of health by connecting individuals to both clinical services and critical social supports.
Under the leadership of the Community Engagement and Wellness Manager, the member will support the development and implementation of community-based outreach strategies that promote FPCSN’s services and increase access to care. This includes participating in health fairs, community events, and partnerships with local organizations such as schools, food pantries, and faith-based institutions to engage underserved populations and recruit new patients into the Network.
The member will also contribute to program development efforts by helping to design and implement health and wellness initiatives that reflect community-identified needs. This includes supporting Community Advisory Councils, gathering feedback from residents, and assisting in the development of the FPCSN Ambassador program to strengthen community voice and participation.
In addition, the member will play a key role in tracking outreach activities and outcomes, maintaining data on community engagement efforts, and supporting continuous quality improvement. By working closely with health center staff, social service teams, and community partners, the member will help ensure that individuals are effectively connected to healthcare, public benefits, and other supportive resources.
Major Duties and Responsibilities
- Member will conduct outreach at health fairs, schools, food pantries, and community events to educate residents about available healthcare and social services.
- Member will recruit new patients into the FPCSN Network by engaging individuals in community settings and connecting them to care.
- Member will provide basic health education and distribute outreach materials to increase awareness of preventive services and available resources.
- Member will assist in planning and implementing health and wellness programs that address social determinants of health (e.g., food access, public benefits, preventive care).
- Member will gather community input through conversations, surveys, and event engagement to inform program design.
- Member will support workshops or activities that promote healthy behaviors and resource utilization.
- Member will support coordination of Community Advisory Council meetings, including outreach to participants and meeting logistics.
- Member will engage community members and partners to increase participation in FPCSN programs and initiatives.
- Member will assist in developing and supporting the FPCSN Ambassador program to expand community outreach capacity.
- Member will track outreach activities, including number of individuals engaged, referrals made, and types of events attended.
- Member will assist with maintaining accurate records and compiling data for reporting and program evaluation.
- Member will help gather feedback from community members to improve services and outreach strategies.
- Member will recruit volunteers and community ambassadors to support outreach events and engagement activities.
- Member will help coordinate volunteer participation at health fairs, workshops, and community-based events.
- Member will participate in group service projects such as community health events, resource drives, and neighborhood outreach campaigns.
- Member will collaborate with FPCSN staff and partners on special initiatives that promote community health and access to services.
