Position Summary

The Wright Center is initiating a program wherein a Community Health Worker will work directly under and with the Community and Agency Referral Coordinator/Medical Social Worker/Director of Behavioral Health Integration/Director of Patient and Community Engagement. The community health worker (CHW) will be responsible for helping to expand The Wright Center's mission of improving the health and welfare in communities . This will be accomplished by the CHW working directly with the Referral Coordinator and other staff mentioned above in the community and clinic settings. This initiative is vital to integrating Behavioral Health within the Primary Care Setting. Currently, The Wright Center does not have a CHW working in this capacity and we are looking to initiate this service line with the goal of having it grow and expand into every community that we serve. The BH CHW will also assist employee health with CPR/AED/First Aide Training.

Major Duties and Responsibilities

The BH CHW will work closely with the Referral Coordinator/Social Worker, Director of Behavioral Health Integration, and other medical and BH providers to provide Behavioral Health Integration and social service resources to Wright Center Behavioral Health patients that are struggling with mental health issues, food insecurities, housing insecurities, and lack of insurance. This role will be a bridge between behavioral health and primary care medicine. The BH CHW will contact patients via telephone or in person at their appointments to provide touchpoints, check ins, and referrals to social services to close the gap in their treatment. The BH CHW will provide support and referrals to those in need of services. The BH CHW will also assist with completing BH outreaches and Integrative Care Plans.

The candidate must complete a child abuse clearance, a health physical and a health attestation form, be up to date on all immunizations and document a negative Tuberculosis test.
