Organization Description and Mission::
CHP is dedicated to improving the health and wellbeing of children through excellence in patient care, teaching and research. The
mission of the Office of Health Improvement Partnerships is to create health among all children and adolescents, and reduce health
disparities, through a population-level approach to physical and emotional health and wellness. We further aim to address the factors
that influence health including access to quality clinical care, health behaviors, socioeconomic factors, and the physical environment by
establishing a prevention-oriented health system, working with community partners to promote policies, practices and systems-level
change where children live, learn and play, strengthening the integration of clinical systems with community-based prevention efforts,
and evaluating process and outcomes to refine efforts and support maximum impact.
